Signed when you Register

Payment Information

A Credit Card is required on file. You may pay with cash/check will be accepted, but a Credit Card is required on file.
Tuition is due by the 1st of each month.

The first installment is due upon registration.

If the credit card on file is declined, there will be a $15.00 late fee applied to the account balance if not paid within 5 days. 

REFUNDS

This registration is for September - May.

If you must withdraw, one month's notice in writing is required in order to cancel your monthly payment. A student is considered registered at SDA until written notice is received. Responsible parties will continue to be charged without written notice of withdrawal.

After January 15th, NO refunds are issued and you will be responsible for the remainder of tuition through May. No refunds are given for classes missed but make-up classes may be taken. 

Medical Emergency

The undersigned gives permission to SDA, its owners and operators to seek medical treatment for the participant in the event they are not able to reach a parent or guardian.

I hereby declare any physical/mental problems, restrictions, or condition and/or declare the participant to be in good physical and mental health. I request that our doctor/physician be called and that my child be transported to the hospital.

COSTUMES

All students have a costume fee per class. If your child does not intend to participate in the Performance, we require written notice by November 1st.

Release of Liability

As the legal parent or guardian, I release and hold harmless SDA, its owners and operators from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant and/or the undersigned, while in or upon the premises or any premises under the control and supervision of SDA its owners and operators or in route to or from any of said premises.

COVID

I understand that SDA cannot be held liable for any exposure to the COVID-19.

If the situation arises that we close due to COVID, all classes will immediately transfer to Virtual Learning (ZOOM).

A VALID CREDIT CARD IS REQUIRED ON FILE.

CASH | CHECK ACCEPTED

TUITION IS DUE ON THE 1ST OF THE MONTH. 

THE FIRST INSTALLMENT WILL BE CHARGED AT REGISTRATION.
